General Comments that relate to this Inspection Ok to issue permit to operate.The following are conditionally approved:-a 3 compartment sink was installed. One section shall be used solely as a handsink and one shall be used only as a mop sink. Label each section.-the ice scoop shall be washed; rinsed and sanitized using 3 separate containers set up on the counter as needed. The only utensil being cleaned at this time is an ice scoop.By 12/1/15; operator will install a separate handsink; a separate mop sink; and an air break for the 3 compartment sink. The hand sink and mop sink may be directly plumbed to the drain line. Install FRP behind mop sink and 3 compartment sink to provide a surface that is smooth and easily cleanable.

General Comments that relate to this Inspection The following items must be corrected prior to permit issuance:1. install 3 compartment sink; must drain indirectly to floor sink. Provide sanitizer and test strips for sink.2. provide soap and paper towels at handsink3. Provide the following information on the vending machine and inside the store: name of business; phone number; water obtained from an approved public water source; type of containers approved for use in the facility.4. Clean and organize back room so that storage containers are protected from potential contamination. Notes:-backflow prevention device in place-ice machine clean; scoop properly stored-ice bags labeled appropriately-bottle flushing station installed-Water shall be tested at point of dispensing at least every 6 months for total coliform and total dissolved solids.
