[4] Facilities to maintain product temperature Reach in freezer holding @ approx 29F so is temporarily being utilized as a refrigerator until the unit can be repaired. Repair is scheduled with contractor and operator shall contact health inspector once the repairs are completed. Twice a day temperature logs are required until the unit is repaired and shall include ambient temperature and at least one food product each time. All food in unit is labeled as if it was a refrigerator and shall continue this way unitl unit is consistently holding 0F.
General Comments that relate to this Inspection All prepped food is labeled properly and staff is aware that food must be served or discarded within 7 days of opening or prep date unless a manufactures use by date overrides that.

[5] Source sound condition; no spoilage Observed numerous items being stored in reach in fridge and make up unit fridge that were past the manufactures use by date or had been prepped well over 7 days prior. (Meat sauce from 10/2; eggs from 10/16; artichokes from 10/14; french toast batter from 10/10; and pineapple from 10/16). All items discarded during inspection. Facility must address this problem and ensure all product is rotated and used or discarded by manufactures use by date or within 7 days of prep date. Items not labeled with prep date were also discarded as prep date was unknown.
[4] Facilities to maintain product temperature Reach in freezer is not holding food at 0 F. Observed it at 36F during inspection. Facility is aware of problem and has been trying to monitor it but some food in the unit has defrosted and it was unknown how long it had not been frozen. These food items were discarded during inspection. Unit must be repaired to ensure it holds all food at 0F at all times or replaced with a new unit. Temperature logs are required twice a day until unit is repaired or replaced. Logs must include ambient temperature and at least one food product. Example logs left with operator.Until the unit is repaired or replaced and food is being held at 0F all potentially hazardous food in this unit must also be conspiciously labeled with the date it is placed in unit to ensure all food is served or discarded within 7 days.
General Comments that relate to this Inspection Hand sink in kitchen clean and stocked. Restroom clean and stocked. Make up pizza unit @ 39F. All food @ 41F or below. Three compartment sink set up with 50 ppm Chlorine. Test strips on site. Ice machine clean. Good scoop storage. Good no bare hand contact procedures in place.Kitchen manager not available to ask about additional CFPM's or fails. Will ask these items during reinspection.

General Comments that relate to this Inspection With the exception of the items below all items from Opening inspection have been corrected. - Replace door gaskets to make up unit near oven. OK to issue permit. Facility will be a risk category 2. Facility is very clean and all cold holding units are functioning properly. Manager has appointment with Sysco/Ecolab on 8/13 to set up chemical dispensing unit for three comparment sink. Ensure sanitizer section of sink is supplied with 200-400 ppm concentration Quat solution or 50-100 ppm Chlorine solution and proper test strips. New facility so 60 days timeline given for CFPM certification. Due 10/12/15.Menu currently limited to pizza. Future plans for chicken wings and burgers discussed. Facility shall contact WCHD prior to implementing these new items.

General Comments that relate to this Inspection Due to delays in signing lease new operators have been unable to complete the majority of the items on the Opening inspection. Operator has keep in touch with WCHD during entire process and shall contact health inspector when ready for a reinspection. In the meantime facility shall not serve food to the public until this facility has been reinspected by WCHD.
General Comments that relate to this Inspection All items from opening inspection have been corrected with the exception of the following: - Replace broken and cracked door gaskets to far left door of glass fridge and middle door of beer fridge. - Install a permanent splash guard between hand sink and ice well to prevent cross contaminationa of customer ice. A temporary barrier is currently in place. Due to delays in signing the lease the new operators were not able to complete the 60 day items. The deadline for these items have been extended until 9/30/15.

General Comments that relate to this Inspection Ok to issue permit. Facility will be risk category 1. The following items must be corrected prior to 5/27/15 at which permit to operate may be suspended: - Reangle drain pipes under three compartment sink so all terminate over floor sink. - Remove duct tape from drain pipes that are being held together and replace; if needed; with ties that are smooth and easily cleanable. - Repair leak at Hand sink. - Deep clean inside of keg cooler and repair piece of weather stripping that has fallen away in left hand corner of unit. - Deep clean in back corner behind keg fridge. - Remove blue tape from speed gun drain lines at far end of bar near keg fridge and attach the drain lines with proper fittings. - Replace broken toilet tank cover to toilet in women's restroom. - Deep clean floor of keg walk-in fridge. This includes but is not limited to removing all old caulk; areas near air vents near ceiling; behind and alongside all equipment. - Remove and replace caulk around hand sink with clear or white caulk. - Reattach drain line from hand sink to wall so the drain line terminated above the flood rim of the floor sink. - Reach in freezer needs to function as a freezer; not as a refrigerator as it is currently functioning. - Deep clean interior of reach in fridge and freezer including all shelves; walls; bottom shlef and top air vents. - Tighten faucet for hand sink near back door. - Repair broken trim around hand sink near back door. - Ensure hand sink is stocked with paper towels. - Deep clean behind front counter. The following items must be corrected within 60 days of opening: -Replace door gaskets to make up unit near oven. -Replace all broken linoleum tiles throughout kitchen and in front of beer walk-in. -Repair broken laminate sections behind front counter.
