    -Numerous items were observed on the floor of the walk in freezer. Ensure all food items are stored at least 6 inches off the floor to reduce the attraction of pests and vermin and to reduce product contamination.*Remove product from the floor of freezer by 1/15/16.

    Notes: Facility was current on PCO maintenance. They are changing PCO as of 01/01/16 to Rentokil North America Inc. Loading dock door that was addressed in previous inspections has been repaired and new weather stripping was added to reduce vermin from entering. New mesh was installed over an exterior opening to reduce vermin from entering.-Back store room was organized. No mouse droppings observed.-Food isles were inspected and no mouse droppings were observed.-Walk in cooler at 41F. Dates good and food stored properly.-Walk in freezer at 6F. Door was open for stocking shelves. All items were frozen.-Ice from Arctic Glacier.-Outside garbage stored properly.-Restrooms were clean and stocked.

    No evidence of vermin activity during inspection. Numerous traps have been placed throughout back storage room and separate area leading to employee restrooms. Area is much cleaner but staff needs to ensure all areas are kept clean of food crumbs and lint and dust as these can cause mice population to reestablish. Threshold under roll up door in back has been improved to add a bristle type brush. Sighting log was kept by staff for 15 days for cookie aisle and no activity observed.

    Observed various areas of back storage room with food debris and packages of food on floor. Also observed various areas of back storage room with dust and debris build up. These areas need to be thoroughly cleaned and kept clean at all times to limit nesting materials and further attraction of vermin. This item will be reinspected 4/15/15 or later. Observed roll-up door to back storage room not sealing completely against floor causing an entry point for vermin. This door threshold needs to seal completely against the floor so daylight is not visible through it. This item will be reinspected 4/15/15 or later and should be in the process of being addressed by that time. Observed additional mice droppings on cookie aisle shelves. All shelves in this section must be cleared off and thoroughly cleaned and disinfected with a 50 ppm bleach/water solution within the next 24 Hours. These shelves then must be monitored daily for further activity with a log sheet being kept. If futher activity is observed the Pest Control Operator and WCHD inspector needs to be contacted so alternate measures can be taken to get this problem under control.

    All items requested to be corrected within 24 Hours from previous inspection have been corrected but futher activity continues to be observed and a few items need to be addressed on a permanent basis (cleanliness of back storage room and roll up door). If these items are not addressed in the timeframes established or WCHD is not contacted to request an extension a reinspection fee of $71 may apply.

    Evidence of vermin (mice) activity continues to observed. One mouse was caught in a snap trap more than 24 hours prior to inspection and still has not been removed. Once observed; dead mice must be disposed of as soon as possible. Once this particular mouse is removed the area around it needs to be thoroughly cleaned and sanitized. This item will be reinspected in 24 Hours. Observed mice droppings in aisle 8 in between Animal Cookies and Smart Water. These droppings look old and should be cleaned up with a bleach water solution. Area should be monitored for further activity and the Pest Control Operator must be contacted if more droppings are observed. This item will be reinspected in 24 Hours. Observed various areas of back storage room with food debris and packages of food on floor. Also observed various areas of back storage room with dust and debris build up. These areas need to be thoroughly cleaned and kept clean at all times to limit nesting materials and further attraction of vermin. This item will be reinspected in 14 days. Observed metal mouse trap in back storage room lying upside down and one trap near front entrance lying on its side; making both of them unusable. Observed other traps lying away from walls as if they had been pushed away by staff during cleaning and not returned. These traps need to be kept in the location they were placed in by the Pest Control Operator to ensure they remain effective. This item will be reinspected in 24 Hours.     Facility continues to have evidence of vermin (mice) observed. According to pest control operator records the facility experienced a time period with very little activity since late January but activity seems to be picking up again. Facility needs to work to address this problem permanently or the permit to operate a food establishment may be suspended. If items are not addressed in the timeframes established or WCHD is not contacted to request an extension a reinspection fee of $71 per reinspection may apply.

    340) Evidence of vermin activity continues to be observed. Significant progress has been made on exterior exclusion activities. Mass trapping has been conducted and interior items have been sealed.The pipe on the south exterior wall exiting from the pillar needs to be sealed/capped.Establish cleaning schedule to include daily cleaning of areas exhibiting evidence of rodent activity (droppings; urine) with a bleach solution.Deep clean trash areas behind front registers.

    Area near trash bin is improved and has been power washed to remove some of the accumlated debris and human waste. Facility should continue to work on this area to ensure it stays clean and free of debris. Facility is working with Waste Management to increase pick ups due to recent cleanings. This item will be reinspected in 14 days.

    Numerous exterior exclusion activities have been completed and other items have been placed on order. The back storage room has been cleaned and store shelves and the underneath of gondolas have been cleaned out. The following items should be completed next: 1. A mass trapping needs to be conducted as per Walgreen's internal Pest Control Policy; beginning today (1/13). The traps need to be located in areas throughout the back storage room; the main store; the fire riser room; the pharmacy; the checkstands and in the ceiling. 2. The metal grate in the fire riser room needs to be sealed with a fine mesh to keep this area from being an entry point for vermin. 3. The holes around each end of the large pipe that traverses across the ceiling in the back storage room need to be sealed. 4. The missing ceiling panel above the battery wall needs to be replaced. 5. The fan on the wall near the walk in fridge needs to be properly covered. 6. Once the mass trapping has been completed the facility needs to continue to follow the Walgreen's internal Pest Control Policy and any items observed and noted on the Pest Control Report need to be remedied as if they were directives from WCHD. This facilty will be reinspected in 14 days to evaluate the progress on the infestation.

    Observed toilet in men's employee restroom not flushing properly. Operator changed women's restroom to a coed restroom temporarily. This needs to be repaired within 48 hours.Also observed a toilet in the women's restroom not working and human feces has been left in the bowl and not flushed completely. This toiled needs to be repaired within 48 hours.

    Observed various shelves and crates being stored in garbage storage area lying against wall of building and debris accumulating around these items. The debris need to be cleaned up and all items stored away from the side of the building in order to eliminate harborage areas for mice and other vermin. Entire back storage area also needs to be scheduled for a power washing to remove other debris and human waste that has accumulated on the back walls of building. This should be completed within 7 days.

    Observed evidence of mice throughout entire facility. Facility has been dealing with this problem for approximately 4 months and the problem does not seem to be diminishing. Therefore the following items must be completed in as close to the following order as possible: 1. Seal all exterior entrances to facility that may be allowing mice to enter the store. This includes; but is not limited to; the following items that were observed during inspection: an opening in the caulk around a concrete pillar near the garbage enclosure; two openings around pipes to the east of the main front doors; front doors need new thresholds as a large gap exits below each; cover grates and holes in fire riser room; ensure drain cover is in place at all times in mop sink. 2. Thoroughly clean all floors under shelves in back storage room. Floors are currently littered with food debris and nesting materials for mice. 3. Thoroughly clean all shelves in main store where evidence of mice has been observed and under gondola bases to remove all nesting materials. 4. Once the above items are completed the facility needs to coordinate a mass trapping event with their Pest Control Operator to eliminate the current population. 5. After the mass trapping has occured all interior holes in the back storage room need to be properly sealed to prevent the mice from continuing to enter the back room. This includes the holes in the Northeast corner where the drywall has broken through and has been patched with foam spray; the holes around the pipes going along the ceiling; etc. Due to the nature of the current infestation; the exterior items need to be sealed; the back storage room cleaned; the top of all shelves in the store cleaned and the underneath all gondolas need cleaned by Tuesday January 13th when the facility will be reinspected by WCHD. Failure to complete these items may result in the suspension of the permit to operate a food establisment at the time of the reinspection.
