Observation: Facility does not have a Certified Food Protection Manager on staff. At least one person with supervisory or management responsibility must be certified and employed by the facility.
- Storage and maintenance of wet and dry wiping cloths
Observation: Wet wiping cloths were stored on top of prep tables not in sanitizer. When not in immediate use the wiping cloths must be maintained in sanitizer to prevent buildup of bacteria.
- Food temperature measuring devices are accurate to ±1°C or ±2°F in the intended range of use
Observation: Walk in cooler thermometer was reading @ 60*F, but the internal temp was between 35-37*F. Thermometer must be repaired or replaced.
- Plumbing system maintained in good repair
Observation: Condensate drain line for the walk in cooler is currently draining into a bucket with a garden hose attached to the end of the copper line. The drain line must be hard plumbed and ran properly to a plumbing fixture (floor sink, floor drain, etc.). Contact a licensed plumber for repair.
- Heating, ventilating, and air conditioning systems design, installation, and cleaning
Observation: Vent covers in the kitchen are dusty throughout the whole facility. Covers must be cleaned.
- Physical facilities maintained in good repair
Observation: Bathroom ceiling tiles still have water stains. Last routine inspection (4/15/14) had noted same violation due to apparent roof damage. Must repair roof damage if existent to prevent contamination.
- Non-food contact surfaces and food contact surfaces of cooking equipment and pans are clean
Observation: The following areas have become soiled and must be cleaned: Pizza oven, wall and floor behind peg board (by walk in drain line), and the dry storage racks.
- Equipment location, installation, repair, and adjustment
Observation: Delfield prep cooler has torn door gaskets (both left doors). Must replace to ensure adequate temp is maintained.
- Established procedures for responding to vomiting and diarrheal events
Observation: Facility does not have procedures for responding to a vomiting or diarrheal incident. Must write a procedure for these incidents and train employees on how to use them.

Observation: faucet on 3 compartment sink has leaks. Repair or replace to maintain in good condition.

Observation: Ceiling tiles in restroom are water damaged after heavy rain on last Sunday. Must repair if there is any roof damaged. Physical facility must be maintained in good condition. Check roof and repair as needed.
- Sanitization methods - hot water, chemical
Observation: Sanitizer in 3 compartment sink was lower than 200 ppm. QA sanitizer must be between 200 to 400 ppm.
- Cleanability of floors, walls, and ceilings
Observation: 1. Missing split ring on discharge line under hand sink in kitchen. Repair. 2.Ceiling tiles in restroom are water damaged and need replaced.
- Common name-working containers
Observation: Sanitizer spray bottles were not labeled. Must label all working container with common name for easy identification.
- Heating, ventilating, and air conditioning systems design, installation, and cleaning
Observation: Vents are dusty throughout facility. Clean more frequently to keep dust free.
- Food storage, prohibited areas
Observation: A bag of yellow corn powder were stored on the floor. Truck came in earlier. All food must be stored 6" off floor to prevent contamination. Correct at time of inspection.
- Posting inspection reports
Observation: Didn't not have most recent inspection report. Must post in area viewable to the public.
- Handwashing signage
Observation: Handwashing sings were not posted on hand sinks (kitchen and employee restroom). Every hand sink to be used by employee must has sign posted reminding employees to wash their hand.
