This is repeat violations.

Observation: Did not have employee agreements for all employees on file. All employees must have an employee agreement on file.

Observation: Did not have employee hand washing sign posted in the employee bathroom. Must post hand washing sign posted at all hand sinks used by food service employees.

Observation: Test strips were expired at time of inspection. Must have test strips that are within expiration date at all times for accurate test result.

Observation: QA sanitizer in the bucket was too weak. Concentration of QA must be maintained between 200-400ppm. Sanitizer made through dispenser. Must fix dispenser to ensure it works properly. May not use dispenser until can dispenser sanitizer at correct concentration.

Pizza at 127 degrees in hot hold unit, temp turned up and now at 140 degrees F, will all e consumed in next hour. Monitor unit closely to ensure holding all PHF at 135 degrees or higher until service.
