[2] Toilet rooms: self closing doors; fixtures maintained; clean; hand soap/drying devices provided; proper waste receptacle Observed toilet tank for restroom in mens room cracked. Toilet in womens restroom has been replaced. This item was supposed to be replaced as part of the opening inspeciton and an extension of 90+ days was granted. These covers were then required to be replaced by 6/19/15 and a voice mail message was left for health inspector by owner that they would be replaced the weekend of the 19th-21st. This item needs to be corrected no later than July 6th or a reinspection fee of $71 will apply for all future reinpsection associated with this issue.

[5] Potentially hazardous food meets temperature requirements during storage; preparation; display; service; transportation Observed baked potatoes cooling in walk-in that were cooked approx 3 hours earlier @ 70F+ wrapped in tin foil. Potatoes cannot be cooled while wrapped in foil. Potatoes unwrapped & cut open during inspection. Facility shall keep cooling logs for all items cooked and cooled until further notice. Observed chicken wings in top of make up unit w/ temps ranging from 47F-52F in top portion of bin as bin was really full. Middle to bottom portion of bin was 40F & below. All food above 40F dicscarded. Facility needs to watch this bin and not overfill it.

General Comments that relate to this Inspection All items from opening inspection except the following have been corrected: - Install FRP on the small section of wall beside and above hood and wall divider to the right of flat top grill.- Replace cracked toilet tank coveres in both restrooms.Operator plans to have FRP completed by Monday 2/23/15 and wil contact WCHD once completed. Cracked toilet tank covers should be completed no later than 90 days routine inspection.

General Comments that relate to this Inspection Ok to issue permit until February 13; 2015 at which time the following items will be subject to reinspection:1) Deep clean and along the sides of both fryers and the stove/grill.2) Install FRP on the small section of wall beside and above hood and wall divider to the right of flat top grill. Finish painting or repaint the FRP to the right of flat top grill.3) Replace wall plate to light switch for walk-in cooler.4) Replace door gaskets to the salad prep station.5) Replace door to the back door o walk-in cooler.6) Recaulk around the base of the toilet in both mens' and womens' restrooms.7) Replaced cracked toilet tank covers in both restrooms.8) Recaulk existing 3-compartment sink or ensure proper caulking on new (planned) larger 3-compartment sink.
General Comments that relate to this Inspection Ok to issue permit until February 13; 2015 at which time the following items should be completed/corrected:1) Seal all the bare wood shelves with an appropriate compound so they are smooth and easily cleanable.
