Permit to operate is reinstated. Both dishwashers are dispensing 100 ppm Chlorine and sanitizer logs have been developed for each machine. Logs are required for each machine twice per shift per Boomtown's internal policy until further notice. All customer glassware must be ran through a dishwasher to ensure they are fully sanitized prior to using them to serve guests.

Observed both dishwashers dispensing 0 ppm Chlorine and sanitizer logs not be kept as requried in previous inspections. Facility was not operating during inspection but it is apparent that the facility has operated every weekend since 12/26/14 without staff checking sanitizer levels on either dishwasher. Dishwasher must be repaired to properly dispense 50-100 ppm Chlorine and sanitizer logs must be kept at a minimum of twice per shift; per Boomtown's internal policy.Recommend rotating the server station dishwasher 180 degrees so that prime controls are on the bartender side and not the server side as the bartenders are the staff members responsible for checking the sanitizer levels.

The permit to operate this facility is suspended as this is a repeat violation from previous inspection and this facility has no other way to wash and sanitize dishes. Operator must contact WCHD to request a reinspection and this facility must not operate until reinspected.

All violations from previous inspection have been corrected: - Dishwasher near waitress station has been repaired and is dispensing 100 ppm Chlorine. - Ice machine has been deep cleaned. This unit is allowed to be used now. - Styrofoam has been removed from reach in cooler. - Floors are much cleaner. * A few areas under counter near waitress station ice bin still need some cleaning attention and duct tape needs to be removed from fridge near small dishwasher.

Observed dishwasher near waitress station not dispensing sanitizer solution. Staff attemped to prime unit but it would not dispense sanitizer and the tube did not appear to have had sanitizer run through it in some time. This unit shall not be utilized until it is dispensing 50-100 ppm Chlorine and sanitizer logs are required for both dishwashers until further notice. Example logs left with staff.

Observed cutting board for garnishes with deep groves in it and is no longer cleanable. Replace this cutting board to prevent cross contamination of food. This item corrected on site. Observed ice machine in waitress area dirty with mold buildup. All ice in this machine needs to be burned down; the unit thoroughly cleaned and sanitized. The unit shall not be used until reinspected by WCHD.

Observed large pieces of styrofoam being used as a false bottom in a reach in fridge. This styrofoam is dirty and not cleanable. Replace this with something that is smooth and easily cleanable.

Observed sanitizer bucket in place but with 0 ppm sanitizer solution. Moist wiping cloths must be kept in sanitizing solution with 50-100 ppm Chlorine or 200-400 ppm Quat between uses to prevent bacteria growth. This item corrected on site.

Observed hand sink at entrance to bar blocked with a metal plate and no stocked with paper towels. This hand sink is not the main sink but should be accessible and stocked at all times. This item corrected on site.

Observed floors and floor sinks throughout bar dirty with syrup; beer; straws; beer caps; etc. All floors and floor sinks need a thorough cleaning and should be placed on a routine cleaning schedule. This includes area in waitress station.

Observed various chemical bottles with no labels indicating their contents. All chemical bottles must be labeled to prevent cross contamination. This item corrected on site.

All the violations observed in this bar have been observed in other bar outlets within this establishment. This facility lacks active managerial control and will be reinspected in 72 HRS. All remaining violations not corrected on site must be corrected within 72 HRS or the permit to operate a food establishment will be suspended.
