    Violations from previous inspection have been corrected. - Ice machine has been cleaned and sanitized. Roof in front of ice machine was leaking during inspection due to rain. Operator is aware that ice machine shall not be used until roof leak has stopped and been patched.

    Observed parts of ice machine still dirty with mold and mildew. Operator states to have an internal cleaning procedure for all ice machines. This machine needs to be thoroughly cleaned and sanitized and must not be used until reinspected by WCHD. Recommend facility keep a log book of all ice machines property wide and when they are cleaned and sanitized. Current practices do not seem to be enough to ensure customers are being served safe ice.

    The following violations have been corrected: - Thermometer in cold holding unit. - Personal items and unused items in front of ice machine have been removed.

Ensure all refrigeration and freezer units are stocked with a conspicuously placed thermometer in order to monitor unit temperature.- No thermometer in the cooler containing the Half&Half creamer and Whipped Creme. A functioning thermometer must be placed inside the unit to monitor the temperature of potentially hazardous foods (TCS foods)

    - Ice machine observed to contain mold/mildew along inside walls and defelctor plate. Ice must be burned or discarded and the interior of the machine cleaned and sanitized before being put back into operation.

Eliminate items not in use and discard broken items.

    Observed speed gun with tape on handle. Remove tape and thoroughly clean and sanitize gun handles to prevent cross contamination. **This item has been noted on two prevous inspections (6/18/14 & 7/3/14) and discussed with management staff on other Boomtown inspections and still has not been addressed. If parts need to be ordered to address this issue then WCHD staff needs to be informed of steps being taken to abate this issue. No attempt to inform WCHD staff has been made. A reinspection fee of $71 will be assessed in order to confirm this item has been corrected.

    Observed storage refrigerators behind bar with dust build up along the top and side walls. Thoroughly clean these units to prevent cross contamination. **This item has been noted on two prevous inspections and discussed with management staff on other Boomtown inspections and still has not been addressed. A reinspection fee of $71 will be assessed in order to confirm this item has been corrected.

    All violation from 6/18/14 inspection with the exception of the above have been corrected. -Dishwasher near waitress station dispensing 100 ppm Chlorine. -Sanitizer buckets in place and all towels in bucket. Bucket @ 100 ppm Chlorine. -Faucet leak has been repaired. -Hand sink faucet has been repaired.

    Observed facility not utilizing sanitizer buckets. Wiping cloths are being left out on counter. Moist wiping cloths must be kept in sanitizing solution between uses to prevent bacteria growth.

    Observed hand sink faucet that comes completely apart and soap dispenser that needs to be permanently attached to a surface to avoid cross contamination.

    Hand sinks stocked. Dishwasher facing main bar @ 100 ppm Chlorine. Good ice scoop storage. *Remove all unnecesary items from sinks in far corner of bar. These sinks should not be used for storage and all food contact items should be stored in a sanitary manner.
