Reviewed with operator to change Quat sanitizer buckets at least once every 4 hours or more frequently as needed when sanitizer water has become visibly dirty. Cold holding units checked OK and within regulations < 41 F. First in; first out method of rotation used. Reviewed handwashing with operators. Gloves do not have to be worn when washing fruits and vegetables.A reinspection is required.Facility has not had employee(s) take and fail one of the recognized CFPM exams in the past year.

The following violation continues to exist:Ice build up on condenser line in walk in freezer. Ice build up is larger in size. Repair leak and clean freezer of ice build up. Spoke to Gary Glenn; maintenace - stated they have worked on the line. He believes it is due to excess moisture in the freezer. Jerry spoke to the head of engineering who will send another maintenance person to look at it. Mike Gilbert; head of engineering stated that if the unit is not repaired by Monday; a new unit will be ordered and installed ASAP. Jerry will email inspector on Monday with update. A reinspection fee will be charged.

Observed several doors bent and not sealing at bottom. Repair or replace doors to ensure a good seal and repair or replace unit to maintain product at 40F and below. Do not use unit until repaired and maintaining product at 40F or below.
