Observed raw meat products and seafood stored above ready to eat and cooked foods. Raw product stored incorrectly. Store all raw foods below cooked and ready to eat foods to prevent potential cross contamination.Re stock shelves so all ready to eat foods and cooked foods are stored properly and not beneath raw meat products in walk in reefer. Reviewed with operator order of raw meat storage in cold holding units.

Observed no sanitary disposable towels in employee restroom. Keep hand sinks stocked with soap and paper towels at all times to ensure proper hand washing.

All cold holding units checked OK. No chemicals stored next to food.Invoices retained for fish sold at grocery store. Do keep invoices and boxes for all fish. Fish is frozen and comes from G.L. Food Wholesale Inc. No damaged food products sold at grocery store. Only prepackaged foods are allowed to be sold at store. Discussed with operator construction requirements for facility if facility wishes to open a snackbar permit and serve open food items and prepare food.

Thermometer missing in cold holding unit. Ensure all refrigeration and freezer units are stocked with a conspicuously placed thermometer in order to monitor unit temperature. Observed thermometers missing in the following cold holding units:1) Reach in deep freezer bunkers. Please put them in a conspicous spot of freezer. They may be velcro to top of freezer doors. 2) Reach in ice cream bunker.3) Reach in freezer next to walk in refer.* All food observed frozen.

Cold holding units checked OK and within regulation.Do store any raw shell eggs on bottom and below ready to eat foods in walk in refer. No chemicals stored next to food; All food observed stored off ground.Bathroom observed stocked and clean.Grocery store observed clean and organized.

This grocery store will be selling only commercially prepackaged foods. No open food items are to be sold at this store.This grocery store will be assigned a risk category 1 health permit. Cold holding units checked OK and recorded < 40 F. Lights properly shielded and adequate lighting.Utility mop sink is adjacent to handsink in restroom.Please provide a thermometer in reach in freezer and a covered trash can in unisex restroom. Install a working paper towel dispenser in restroom.OK to operate; OK to issue health permit. OK for operator to stock grocery store. Please post official health permit in a conspicuous spot in store once received via US post office. An annual inspection will be conducted within 90 days. This inspection will be unannounced.
