    - Operator offering for sale individual unlabeled product not intended for individual sale (string cheese sticks). All food product must be sold in its original labeled package for trace back purposes and in order to show ingredient and nutrient information.- Operator offering bagged ice for sale without labels. Ice is prepared on site. Bags of ice must be labeled with the store name; address and phone number where made for trace back purposes.

    - Raw beef (prepackaged hamburger) must be stored on the bottom shelf of the cooler to prevent any cross-contamination from a leaking package onto other food which might not get cooked.- Observed a slight accumulation of mold on the leading edge of the plastic drop chute in the ice machine. Clean and maintain clean.

    Install thermometers in the food deli refrigerator and in the tv dinner/frozen pizza freezer where missing. Freezers must be held at 0F or less and refrigerators at 41F or less.

    OBSERVATIONS:- Muffins sold individually must continue to be handled with paper/plastic to prevent hand contact - As stated by operator. Be advisedl; bare hand contact with ready to eat food is prohibited.- Restroom stocked and operational with hand soap and paper towels; hot and cold water under adequate pressure with no leaks.- Ensure that expired or damaged product is promptly removed from sale and discared.- Permit limits operation to the sale of commercially pre-packaged food items only. Ice may continue to be produced and sold if properly labeled as indicated above.

    All milk and dairy products are to be stored in the propper location. Milk was observed to be stored on the floor of walk-in refer at the time of inspection.

    Observed ice scoop on dirty counter; ice machine is dirty and requires proper and complete sanitizing and cleaning.

    Operator stated "they clean items with windex and towel" when asked. Must discontinue cleaning with windex and use correct sanitizer solution to clean Ice-machine and food service counter with muffins; microwave; and soda fountain.

Discontinue use until unit has been properly cleaned and sanitized. Re-inspection required and must utilize solution with at least 50 ppm bleach.

    Few canned food products noted with expired dates; two cans noted with dents in seam; and one bulged can of salsa product. All these items were removed from shelves to return to provider or to discard.Packages of raw ground beef noted on shelf above packaged ready to eat foods in refrigerator. Must keep always raw meat products below cooked or ready to eat products; corrected.

    Drain pipe of recently installed ice machine is directly connected to building's sewer pipe. Drain pipe must be provided with proper air gap for indirect waste connection.

    Two racks full of trash bags and cardoard and packing materials found being stored inside at back of establishment. Dumpster is currently full; dumpster to be emptied tomorrow per operator. Trash must not be stored inside establishment. Larger dumpster or more frequency of trash pick up service would be required should accumulation of trash inside establishment recur.
